✔️ Initial 90-minute Nutrition & Training Video Call (first month): • Macronutrient and fluid requirements • Individualized nutrition education • Realistic, measurable goals with target dates • Posture and movement assessment to identify overactive/underactive muscles • Goal setting and plan design based on your level, available equipment, and schedule ✔️ Monthly 45-minute Nutrition Video Call: • Progress review, macro adjustments, and individualized recommendations ✔️ Training App (24/7 access): • Your routines 100% personalized in the app, updated every 8 weeks • Demonstration videos for each exercise • Tracking of weights, reps, and progress ✔️ 1:1 Tracking and Support in the App: • Weekly check-ins via chat for questions • Send exercise videos through chat for feedback and corrections ✔️ Designed to work both in the gym and at home (depending on available equipment) ✔️ Investment: • Startup Fee: $225 (one-time at the beginning) • Monthly Membership: $150/month, automatic payments *THIS PLAN DOES NOT INCLUDE PRINTED MENUS/DIETS ✔️ Medical Clearance & PAR-Q+: All clients must complete a PAR-Q+ (Physical Activity Readiness Questionnaire) before starting their training program. The PAR-Q+ helps identify potential health risks or physical limitations to ensure safe exercise. You will receive an email with the PAR-Q+ after placing your order. Please complete it and return it signed. ⚠️ If you have any chronic or pre-existing medical conditions, you must provide a signed medical clearance from your doctor confirming that you can safely exercise and specifying any necessary limitations. Your personalized training plan cannot be created without these documents.

⚠️ All memberships are billed monthly until canceled. You may cancel at any time, but NO refunds will be issued for services already provided. If you received a service (e.g., video call) before your monthly billing date, you are responsible for completing that month’s payment to cover the service delivered. Scope of Service: Nutritional guidance provided by SustainaVal LLC is for educational and informational purposes only. It is not intended to diagnose, treat, cure, or prevent any disease. Medical Care: If medical treatment is needed, you acknowledge that you must have your own health insurance and seek care from a qualified healthcare professional. Liability: You understand and agree that SustainaVal LLC is not responsible for misuse, misinterpretation, or incorrect application of the information shared during nutritional counseling sessions. Privacy Notice: We respect your privacy and protect your personal information. We collect contact information, health and nutrition data (for MNT services), and training/fitness progress (via ABC Trainerize). Your information is stored securely, used to provide personalized services, and is not sold. By using our services, you consent to the use of ABC Trainerize for fitness tracking purposes only. Indemnification: By using our services, you agree to release and hold harmless SustainaVal LLC from any liability and to indemnify us against any claims, demands, or legal actions arising from the use of the information or services received.
